I filled it in but the last question strikes me as useless:

Are you happy to subscribe to web based services or purchase iPhone applications as long as they are reasonably priced and offer a desirable and unique service?

Who isn’t happy to subscribe to something that they both desire and which is reasonably priced? It would have paid to be more specific as I don’t think the result you get from this question will be statistically useful.
